Physiatry and Neurology
After the patient is admitted to the rehabilitation unit, his or her care is directed by a physiatrist, a physician who specializes in physical medicine and rehabilitation, or a neurologist, a physician who specializes in neurological conditions (i.e. a stroke, multiple sclerosis, Parkinson's Disease, etc.). The physiatrist or neurologist will prescribe and manage the rehabilitation plan while keeping the primary physician informed of the patient's progress.
Patient and Family Goals
Patient's preferences, individual goals and needs are foremost in the development of treatment plans. Family involvement and education are also key elements in the process of recovery. When appropriate, family members are encouraged to observe and/or participate in treatment sessions. There is also a formal family training offered when needed.
Patients are generally discharged when their rehabilitation goals are met. Discharge is coordinated to ensure a smooth transition to home or other setting. When appropriate, outpatient therapy or home health care may be recommended for follow-up care.
The program strives to enhance the quality of life and dignity of the clients it serves, promoting discharge to an optimal level of independent living. |
